Hawaii's autism services are shaped by two big realities. The first is Luke's Law — Hawaii's 2015 autism insurance mandate — which requires most health plans to cover autism diagnosis and treatment, including ABA. The second is geography: services are heavily concentrated on Oahu, and families on the neighbor islands often face real gaps, sometimes traveling inter-island by plane to reach specialists. Hawaii has strong bones — the Department of Health's Early Intervention Section, Med-QUEST Medicaid with a robust EPSDT benefit for children, and a Developmental Disabilities Division I/DD waiver — but the Oahu-centric distribution of providers is the honest central challenge.
